  I started work as a Social Prescriber for prescription, plus a Crawley Community Action Project in 2016. I am now working for the Crawley Care Collaborative PCN which includes the following practices in the Crawley area: Bridge Medical Centre, Langley Corner Surgery, Leacroft Medical Practice and Southgate Medical Group. Many things can affect your health and wellbeing. Clinicians tell me that a lot of people visit them feeling isolated or lonely or stressed by work, money or housing related issues. Sometimes it’s the stress of managing different long-term health conditions. That’s where social prescribing comes in, it starts with a conversation. It might be the conversation you’ve just had with your clinician or with another person in the practice team. They will refer you, to me your practice social prescribing link worker, I can support you in finding appropriate low cost or free help or advice.
